How Tadalafil Works

Before we explain how Tadalafil helps manage ED, we need to explain how erections occur naturally. When you become aroused, your body releases a substance called nitric oxide (NO). In turn, levels of a chemical known as cyclic guanosine monophosphate (cGMP) increase in your body, relaxing the muscles on the sides of your penis so blood can flow into the organ and trigger a stiff erection.

Abstract

The efficacy and safety of sildenafil citrate (Viagra) has been evaluated in a randomized, double-blind, placebo-controlled clinical trial in patients with erectile dysfunction. In the current study, we evaluated the efficacy of tadalafil citrate in patients with mild to moderate erectile dysfunction. We evaluated the response to tadalafil at the end of three treatment regimens in comparison with a placebo. We assessed the incidence of adverse events in the treatment arms. After treatment, there were significant differences between the tadalafil citrate groups in all three efficacy measures, with the efficacy in the tadalafil citrate group being significantly superior in the tadalafil citrate group than in the placebo group. Tadalafil citrate did not significantly improve the erectile response to a dose of sildenafil. The tadalafil citrate effect was not different from placebo in the number of adverse events. The mean change in the number of adverse events over three treatment regimens was 4.5% in the tadalafil citrate group and 3.3% in the placebo group. The mean change in the severity of adverse events in the tadalafil citrate group was 1.4% at day three and 0.7% at day five, and the mean change in the number of adverse events was 1.8% at day three and 1.3% at day five. The mean change in the number of adverse events in the placebo group was 2.0% in the tadalafil citrate group and 1.0% in the placebo group. We conclude that the use of tadalafil in patients with mild to moderate erectile dysfunction is safe and well tolerated. Tadalafil citrate improved the erectile response to sildenafil. However, it did not significantly improve the number of adverse events. A placebo-controlled clinical trial is warranted to evaluate the safety of tadalafil treatment in the treatment of erectile dysfunction.

This is an open-label, three-center, single-center, parallel-group, randomized, placebo-controlled clinical trial evaluating the safety of tadalafil citrate and sildenafil in patients with mild to moderate erectile dysfunction. Patients with mild to moderate erectile dysfunction were evaluated on an efficacy endpoint of the International Index of Erectile Function (IIEF) scale. The primary endpoint was to assess the change from baseline in the IIEF score from baseline to the end of treatment. Secondary endpoints included the change from baseline to week one of the IIEF questionnaire. The tadalafil group was treated with sildenafil, and the placebo group was treated with placebo. The tadalafil citrate and placebo groups were evaluated on an efficacy endpoint of the International Index of Erectile Function scale (IIEF) scale. The primary endpoint was to assess the change from baseline to week one of the IIEF questionnaire. We also evaluated the change from baseline to week one of the IIEF questionnaire. This study should serve as a validation set for the comparative efficacy of tadalafil and sildenafil in patients with erectile dysfunction.
